Meniscal Root Tears can be managed Operatively and Nonoperatively

  • Meniscal roots are a form of the meniscal tear in the knee, wherein the tear extends to the anterior or posterior meniscal root attachment to the central tibial plateau. Often, meniscal roots are radial tears extending into the meniscal root. Meniscal root damage can occur mostly in a multi-ligamentous knee injury, traumatic hyperflexion, or chronically, secondary to degenerative changes. The sufficient vascularization of the meniscal root allows arthroscopic techniques to repair lesions, with either suture anchors, or transosseous sutures.

Treating Meniscal Roots through Operative and Non-operative Procedures

  • It is possible to treat meniscal roots through both operative and non-operative procedures. Let us look at the non-operative method first. The non-operative method comprises symptomatic treatment with NSAIDs, activity modification, or an unloading brace. Patients who cannot undergo surgeries, like those with multiple comorbidities, who’ve got an advanced degenerative joint disease, or aged, can go for non-operative treatments.
  • However, if there are no improvements even after three months of the non-operative treatment, and if the old symptoms persist, the patient qualifies for an operative treatment, based on MRIs showing minimal osteoarthritis.
  • Now, let us look at the operative counterpart. Operative procedures limit themselves to short-term symptomatic relief, and with unknown consequences in the long run. They are applicable for chronic tear cases with symptomatic grades 3 or 4 chondral lesions, and in cases where the non-operative management did not work.
  • The surgical repair technique involves suture anchor technique via a posteromedial portal, or repair through an anterior approach engaging an arthroscopic flip cutter, and button fixation with an anterograde suture passage.
  • Through the operative alternative, meniscal root treatment helps you restore the hoop tension of the menisci and results in better clinical and radiographic results up to a couple of years after the surgery.
